How I Started a Profitable,
Patient-Focused CCM Program
Physican leaders at a mid-sized nephrology practice knew they wanted to implement Chronic Care Management (CCM) back when Medicare began funding the program in 2015. The practice's 7 physicians and 3 nurse practitioners had shared goals for CCM:
- Deliver superior patient care: slow the progression of Chronic Kidney Disease progression and keep patients off dialysis
